• Certification Details
• Must be AHA BLS certified
• Must be AHA ACLS certified
• Must have current licensure and primary source verification within 30 days prior to start
• NIH Certification required
• EKG Exam required
• MMR vaccine or Positive IGG Titer
• Fit Test annually
• Hepatitis B vaccination series completion or declination documentation
• Influenza Vaccine annually, no exemptions accepted
• 2 Step PPD or Negative IGRA required
• Varicella vaccination or proof of immunity
• Tdap vaccination valid for 10 years
• 12 Panel Drug Screen required
• Ascension Badge Photo
