PRIMARY PURPOSE OF POSITION: Provides prescribed medical treatment,
medication, and personal care services to nursing home residents to
meet individuals spiritual, physical, emotional and social needs
consistent with the plan of care, Standard Nursing practice, scope
of gerontological nursing procedures and all applicable
regulations, codes, policies and procedures
CUSTOMERS/CLIENTS SERVED BY POSITION:
Board members, Staff, Residents, family members, guests, vendors
and other outside clients served by Ovation Communities.
Qualifications:
Experience: N/A
Education: High school diploma or equivalent
Graduate of an accredited school of nursing
Other: Demonstrates strong communication and relationship building
skills with residents, and is able to explain residents needs to
others as needed for resident advocacy
Specialized training or experience in geriatric care or gerontology
is desirable
Ability to work as part of a team
Ability to manage multiple projects
Must maintain complete confidentiality and discretion.
Read, write and speak the English language.
JOB DUTIES:
Receives verbal report and communicates all pertinent resident care
information to the CNAs on assigned unit or area.
Visits each resident to assure the appropriate cares are being
provided, reports any problems to the manager/supervisor.
Administers and documents medication and treatments, etc. within
the parameters of the LPN licensure and according to the care plan.
Reports any changes to the manager/supervisor.
Reports significant changes in residents conditions immediately to
the manager/supervisor and notifies appropriate people as
directed
Maintains timely, accurate written charts, documents, records as
required by law and/or regulation policies and procedures. Charting
must be informative, descriptive or nursing services provided,
residents responses to cares and a notation when
families/physicians are notified of significant resident
changes.
Review and revised care plans as changes occur to reflect the exact
condition of each resident
Completes compliance rounds or resident rooms to assure a clean,
orderly, comfortable environment. Makes adjustments based on
individual needs.
Appropriately identifies problems, approaches and goals on
residents care plans and discusses them with the CNAs daily after
compliance rounds.
Writes timely nursing summaries that reflect residents conditions,
cares being given, restorative/rehabilitation interventions, the
movement toward appropriate goals and acceptable approaches.
Participates in resident assessment on admission, on-going resident
care/family care conferences and discharge planning as required
Makes rounds with Medical Director, attending physicians, nurse
manager, DON as delegates
Assures that all resident rights are maintained at all times.
Reports and violations or suspected deviations according to
policy
Is aware of and demonstrates proper hand washing, isolation
techniques, universal precautions, good handling, linen handling
and cleanliness or resident areas.
Demonstrates prevention and intervention used in caring for the
confused, wandering or combative resident
Participates in primary are nursing as assigned on each
unit/floor
Performs other duties as assigned by supervisor(s), DON or
Administrator
Intervention Intervention is the nursing action to implement the
plan of care by directly administering care or by directing and
supervising nursing acts delegated by RNs.
Accept only patient care assignments which the LPN is competent to
perform.
Provide basic nursing care.
Record nursing care given and report to the appropriate person
changes in the condition of a patient.
Consult with a provider in cases where an LPN knows or should know
a delegated act may harm a patient.
Carries out all duties in accordance with the Jewish Home & Care
Center mission and person-centered care philosophy.
Demonstrates knowledge of and respect for the rights, dignity, and
individuality of each resident and facility information.
Appreciates the importance of maintaining confidentiality of
resident and facility information.
Demonstrates honesty and integrity at all times in the care and use
of resident and facility property.
Able to understand and to follow written and verbal direction. Able
to effectively communicate with staff members and residents through
verbal and/or written means.
Able to interact effectively with all residents.
Knowledge of emergency and disaster procedures of facility.
Able to locate nearest exit, to understand and respond to written
or verbal instruction in case of emergency.
Sufficient mobility, and strength to move freely through the
building, to assure resident safety at all times and to assist,
transfer, or otherwise move residents out of danger in case of
emergency.
Demonstrates respect for co-workers and responds to needs of
residents by complying with facility policies on attendance and
punctuality and dress code.
Able to arrive and to begin work on time and to report for duty as
scheduled on all shifts, weekends, and/or holidays.
Working knowledge and ability to comply with facility policies and
procedures for workplace safety including infection control
procedure, application of universal precautions for blood borne
pathogens, use of personal protective equipment and handling of
hazardous materials.
Demonstrates an ability to prioritize tasks and
responsibilities.
Demonstrates the ability to complete duties and projects within an
allotted time.
Able to respond to change productively and to handle additional
tasks and projects as assigned.
Able to carry out the essential functions of this job (without
reasonable accommodation) without posing specific, current risk of
substantial harm to health and safety of self and others.
Must be patient, respectful, courteous, tactful, understanding and
exhibit a warm and cheerful manner in dealing with residents,
families and visitors
Consistent and regular attendance on the job
Is aware of and follows existing lines of communication and
authority
Participates in and attends departmental meetings, in-services, and
is responsible for maintaining license as dictated by the State of
Wisconsin
Maintains confidentiality of all resident, department and
organizational information
Assures all residents rights are maintained at all times. Reports
abuse or neglect to appropriate staff and are aware of how to file
a complaint
Follows safety, security, hazardous materials policies and
procedures. Performs all tasks to assure resident and personal
safety
Performs all resident cares as assigned and according to JHCC
nursing department policies and procedures
Perform other duties as assigned.
PHYSICAL DEMANDS:
While performing the duties of this job, the employee is frequently
required to sit and to use hands to file and type often.
Occasionally the employee will be required to stand and stoop,
kneel, crouch or crawl. Requires close viewing of computer screen.
Excellent verbal communication skills and excellent vision and
hearing required. Must be able to lift, carry, push, or pull 25
pounds or more occasionally. The employee must occasionally be able
to perform tasks which require steadiness/fluidity of motion.
*Occasionally may be exposed to chemical irritants, loud noises,
and blood splatters.
